- Jack is a worthy & well-rounded doctor type who needs a good run around every now and then. Other character some miracle-maker whom he'll meet/has met on the island
- he's still fat because this is american tv and it's therefore v pc (and of course it's all in his genes...) a few weeks of mangoes & fish won't change that

He explained in season 1 why he's still fat - he is losing weight like the rest of them, but since he has a lot of weight to shift, its gonna take more than a few weeks without decent food.

I think hurley still being fat is just another of the strange island things. Its a bit like Locke regaining the use of his legs. (Or Kate and her endless supply of eye liner 
)
