Help my daughter wont poo apart from in a nappy

Hi, Im sure there are other parents who have had problems with there children not wanting to poo on the toilet however I have a deep feeling with my daughter we have gone beyond the "norm" as it were, she potty trained herself at the age of 18months as in she refused to wear a nappy to wee only on the toilet or potty, when it came to poo however she would only go in her nappy, we left it until she was approx 2years 7 months and started trying to get her to poo on the toilet or potty, well she wont do it, we have tried everything you can ever imgaine, we have done reward charts, bribary, sitting her on the toilet with a nappy on (she refused and screamed) removing nappies completely, showing her all the family going to the toilet etc etc it just doesnt work, she holds it until she is hopitlaised, which is many occasions, one time she held it in so long she nearly blew her bowel up the hospital had her on intensive care! They cant seem to suggest anything, we try and ignore it now, she wont poo in her knickers even only in a nappy. She has serious bowel problems now due to this and the hospital said for her health we need to get her to at least sit in the squatting position to go and she wont. I know I sound a weak mother, my oldest daughter with autism I managed to get her potty trained fully so what am I doing so wrong, I really am desperate! Please any suggestions,

have you been prescribed laxatives at all for her?
it seems to be a common course of treatment for this type of thing. take her out of nappies, and give laxatives so that she can't hold it in and has to go?

have you tried placing a nappy onto a potty/toilet and letting her go on that? sitting her on the toilet whilst wearing a nappy so that she gets used to doing it on the toilet even though it's still in the nappy?

Hi, she takes laxatives daily however these just make is so she can go otherwise she wouldnt go at all, her anal hole is far too small for the pooh to come out. We have tried sitting her on the toilet with a nappy on and over and she has a panic attack where she will scream and scream and scream then hold her breathe until she passes out.

I dunno whether this is gonna make you feel better or worse but here goes...

My DS was exactly the same, wees were sorted day and night but he would only poo in a nappy or pull up...he started to get issues with constipation..so i just gave up and let him do it in a pull up..he got himself into a routine where he'd go in a pullup every evening so it was never an issue at nursery

When he was 4 (!) he had to go to the doctors for some other reason and i threatened to tell the doctor..he agreed he would sit on the toilet and do it in a pull up so we agreed to compromise..anyway about a week after this he started doing it on the toilet

For a while he would only go on our toilet and it was a big event when he did it on a strange one!!

But the upshot of all this is that he now does everything normally and is none the worse for the nightmare time in the past
