"Accidents" outside the litter box.

Our year old boy Dexter keeps pooping outside the litter box. There is no pattern to his behaviour and he does it as and when he see's fit.
Sometimes he goes in the box with no issues and some times he just wants to make my day akward.

No matter now clean we keep the boxes, how far apart we keep them from each other or how deep the litter in the boxes is, he still goes outside the box... right it front of it. We have even tried taking the hood off of his box so he can see his surroundings incase he was feeling nervous about that. Nada.

He gets on well with our other cat Pip and they have both been snipped. They are not territorial with one another and Pip has no issues using his own box every time.

I really don't know what else to do here. I have tried putting the little packages he leaves in the box so he see's where they belong (but he knows that's where he is supposed to go, he does quite often.)
I have tried spraying with water, I have tried yelling 'no' when I catch him in the act. I have even tried putting him in the box when he is showing signs he needs to go ( scrapping the floor outside the box etc.)

I am stumped.
Any advice at this point can only be good advice.

I have been told you should have one litter tray per cat and if they have issues to get another one. So if you have 2 cats you would need 3 litter trays.

If he is a large cat! 5kg plus he may need a larger litter tray. We discovered when our cat was having issues he was standing in the litter tray but his bum was not over the litter tray.

But that's the problem they each have their own tray, and they both use their own. It's only once and a while that he decides to break the rules. He goes in the tray most days. I just don't get why he is being a pain in the bum once or twice a week.

Are they wormed, could he beunwell. My cat has been known to jump into the kicthen sink, look me in the eye and pee when he had cystitis.

Yeah Worming is up to date and they are eating and drinking fine.
No changes in behaviour and just as cuddly as ever. I am baffled. I just don't get why it only happens here and there.
I would get it if it was daily but it's like he is doing it just to keep me on my toes. lol.
