My 10 year old's speech is going downhill

So, my 10 yo has always been a bit of a difficult talker. It's always been as if his brain is working way too fast for his mouth and he's struggling to get all his ideas out. He is only an year younger than his brother, who always used to speak for him when they were very young.

Anyway, he has had speech therapy through (a US) school and was signed off at the end of term last summer. In the last few weeks, I feel as if his speech has got worse. He isn't really stammering, it's more like his thoughts seem really jumbled and he just can't get it all out. For example, this afternoon we went to see a movie and he had this really great idea how the problem in the movie could have been fixed right from the start, but it took him about 3 minutes to finally articulate what he meant. I said to him, "Hey, take a breath, think about what you want to say and then tell me" but he was still struggling a little bit.

I'm taking him to see our Pediatrician next week anyway, but wondered if any of you have any experience of this?
