But you click the link, and fill in the form, and THEN get sent the survey. The clicky link usually IS the survey.

Ok so I've filled in the survey. Question though, re the prize, presumably the winner will be asked what size they require before it it sent out? As my DD is a big tall girl, bigger than some who are a year older, for example.

Thanks to everyone who has added their details so far :) Since I download everyone's details and then email the survey, there will usually be a slight time lag between when you fill in the form and when you are emailed the survey.
