Deed Poll mess!

When DS1, now 19, was 3, I had his name changed by deed poll. I also changed mine at the same time. So as not to drip feed, my parents divorced when I was a baby and I took my stapdads surname but it was never legally. When ds was born I was young and stupid and registered him with his fathers surname and his dead uncles name as a middle name, which I was uneasy with at the time. Ex left us when ds was 3 months old and at that time there was no automatic right to PR with his name on the birth certificate. Ex kept taking me backwards and forwards to court for one thing and another so, while I was dealing with a solicitor I decided to change both of our names legally. When this was done the solicitor asked ds what his name was and ds answered missing out the uncles name, which I was secretly happy about, the solicitor then said 'well that is what your name will be from now on' and did the paper work.
It was only as ds got older that I realised that the document made no reference to ds's birth name. He has recently tried to open a new bank account but needed his own deed poll, as he has lost his passport and has not got a driving licence. My mum, who he lives with, went onto the deed poll website, filled in the forms and ordered 15 extra copies, which cost over £60. This was in November, she is convinced she typed in the middle name. Today ds took them into the bank and it was pointed out that the deed poll certificate did not have the middle name on it, neither mum or ds had noticed, even though my mum had put it on the application form, the bank refused to open an account for him saying it invalid!
I have just had a look on the website and to have it corrected it is going to cost the same again.
It seems a bit strange to me that the passport agency accepted the previous one with no mention of him on but his new name but the bank won't accept this one. It is only for a basic account with no overdraft facility.
Also does anyone know if we have any come back with the deed poll service for the mistake? He really needs to sort this out soon as he has just started an apprenticeship and can only be paid into the bank.

Just type it out afresh but put the correct name in this time. It's isn't a magical document you know.
