Not ovulating, no periods, but FSH and prolactin all normal? What could it mean?

I came of BC 1 yr ago, and have had no periods since. I also have a history of anorexia but have been healthy for 5 yrs now. My doc has just done blood test, which reveals NORMAL FSH, oestrogen and prolactin, yet provera test has not made me bleed!

What could be causing my lack of periods/ovulation...we didn't expect these blood results. Can anyone share any personal experiences or give advice? I don't see my doc for 2 weeks and am very confused!
