Dosage help

The nurse who was originally asked to start my DS on a heart medication queried the dosage and wouldn't give him the full dose to start with as she felt it was too high. She was overruled by the consultant in the end who had read a note from a telephone conversation another doctor had with cardiologist (who then went on leave and I am waiting to hear back from). Anyway this has stuck with me. I have looked at the dosage instructions and it is as was said in hospital 1mg per kilo of weight. My DS is 7 kilos so has 7 mg but has it three times a day. I am trying to work out if he should be having the 7mg spaced over three doses as he actually has 21mg in total a day. Can anyone help?? The official dose guidelines are below:

Oral: Children: Initial: 0.5 to 1 mg/kg/day in divided doses every 6 to 8 hours; titrate dosage upward every 3 to 5 days; usual dose: 2 to 4 mg/kg/day; higher doses may be needed; do not exceed 16 mg/kg/day

Sorry, I don't know but bumping for you. What about an emergency GP appointment tomorrow morning as this is obviously really important. Hope you get it clarified soon.

Reading that it looks like it can go as high as 16mg per kg of weight?

I would phone the consultant who prescribed it to check though if you are concerned.

Thanks for the replies. It wasn't an emergency as he is not on the max dose for his weight but I do want him on the minimum dose possible. The side effects aren't nice for him.

I am waiting for the consultants to get back off leave for a definitive answer but it looks like the dose could come down for DS.

Another member of the consultant's team should be able to advise - can you ring the consultant's secretary tomorrow?

It says the usual dose is 2-4mg/kg per day and your son is having 3mg/kg per day- sounds about right? But of course check with the Dr for clarification

A wild guess it's frusemide and black treacle is spot in with what she has said.
Ideally should be every 8 hours I would go for 10pm, 6am and 2pm as my schedule, but I go to bed at 10pm.
