Positive SPT and negative Arah 2 - peanut challenge

Hi my DS1 has had positive SPT for peanuts and many tree nuts, results ranging from 6-8mm. The hospital did a component test and the arah2 test came back negative. I ate peanuts and tree nuts when pregnant and breastfeeding. When he was born he was constantly showing allergic symptoms such as red watery eyes, eczema and hives. However he was also allergic to wheat, milk, egg, rice and banana, all of which I was eating too so there is no way of knowing if he was reacting to nuts.

The hospital have referred him to another hospital to see about the possibility of a peanut challenge. The dietician said she is confident based on the negative test that he won't react to nuts.

Has anyone else been in a similar situation. I'm still very nervous about avoiding nuts as we have no epipens and just carry piriton.

peanut skin prick test is generally considered very accurate, is the hospital you are being reffered to have a pead allergy clinic?

fingers crossed that he doesnt have peanut allergy, and that he outgrows the rest of the list in future!

Thanks eragon.! The guy I'm going to see is a paed consultant specialising in respiratory conditions and allergies.
My understanding of the arah2 test is that it shows he is unlikely to have an anaphalytic reaction but I could be wrong.
I can't understand why he would have a positive SPT (and strong positives, the poor child was going mad with the itch from the wheals and they appeared before I had pulled his sleeve down), and not react to the allergen when eaten.

Hopefully I won't be waiting too much longer for the appt.

Lima it's so positive to hear that you ds has outgrown allergies. This is what I (and everyone else on here!) dream of. My dd is allergic to milk eggs and nuts and we go back in June to the clinic. Dd is 3.

Hi, he is 3.5yrs. He was diagnoised at 5 mths old. Thankfully he never had a severe reaction just bad eczema and outbreaks of hives. He outgrew banana, rice and wheat at about 18 mths and milk by 2.5 yrs. His egg is the worst and he breaks out in hives from even traces of hidden egg. His results from egg are still fairly high ( rast is 47, SPT was 6mm). Its very manageable and if he never outgrows egg I think it will be something that he will live with with little effect on his life. The nuts, as everyone knows, is a different story so I'm so eager to get a challenge done. The prospect of him not being allergic or at least knowing he won't have an anaphalytic reaction is unbelievable brilliant.

Lima that's so great - I will continue to hope that my dd has a chance of outgrowing the allergies. She hasn't had a reaction for over 18 months - a couple of tiny spots which I couldn't really claim to be hives maybe 3 times since then but it's hard to know if they have been allergy related as she has such sensitive skin (I did too as a child and also she is a red head)

I think that due to the positive SPT but negative component test then possibility is still there that he will react, but that it won't be an anaphylactic reaction.

fingers crossed for you though that he doesn't react at all!

Component testing is all still pretty new, will be interested to hear how you get on, and what they advise you to do following the results of the challenge. Will they continue to monitor him.

If a 6mm weal from a SPT is fairly high, does that mean a 9mm weal for milk and a 12mm weal for tree nuts and the same for peanuts is really high? And a 15mm weal for pineapple?

And what is an Arah2? And rast?

My DS3 is allergic to tree nuts & peanuts, milk, soy & pineapple. The only tests he has had are SPT. He is 2y3mo.

His SPT for soy was 6mm - the Paed allergist says he is confident that DS3 will outgrow his soy allergy. I think he possibly will eventually, as that reaction when he comes into contact with it isn't as bad as his other reactions.

He also thinks he will outgrow his milk allergy by 5yo. Tbh, I don't think he will, as his reaction is anaphylaxis same as nuts, peanuts and pineapple.
